Ad Calendars	Prices include up to six lines of type. Additional lines are $2.00 each. Maximum of three additional lines.

Color Ink
Envelopes Invoices Tithe Envelopes	Black ink is included in the prices shown. To substitute a STANDARD color ink for black add $16.00. To substitute a PMS color ink for black add $35.00 Available standard colors: Dark Blue, Light Blue, Red, Green, Brown, Yellow, Burgundy or Purple.
Raffle Calendars Raffle Tickets Sheet Tickets	Black ink is included in the prices shown. To substitute a STANDARD color ink for black add $70.00. To substitute a PMS color ink for black add $105.00 Available standard colors: Dark Blue, Light Blue, Red, Green, Brown, Burgundy or Purple.

Custom Name And Address
Employment Forms	Your company information can be printed on the employment form, maximum of six lines. $15.00 set up fee. This charge is incurred on the first order only.
Envelopes Invoices Registration Forms Work Schedule	Prices include up to six lines of type unless otherwise noted. You can include additional lines at $2.00 each. This charge is incurred on the first order only.

Stapling / Booking
Invoices	$5.00 setup + $1.50 per book
Raffle Calendars Raffle Tickets Sheet Tickets	Books of 5, 10, 20 or 25 are 8¢ each. + $5.00 setup fee Books of 50 are 10¢ each. + $5.00 setup fee Any other number of tickets to a book, 14¢ each. + $15.00 setup fee

Miscellaneous Services
Custom Indicator Boxes	Invoices	Customize these boxes with up to 7 indicators for $8.00 or you can use the standard indicators at no additional charge. This charge is incurred on the first order only.
Padding with chipboard backing	Invoices	Set up $6.00 plus $.25 per pad, maximum of 200 sheets per pad
Perforating	Invoices	1 or 2 perforations: $25.00 setup + $21.00 per 1000 pieces 3 or 4 perforations: $35.00 setup + $25.00 per 1000 pieces
Numbering	Raffle Tickets	A $15.00 setup fee will be charged for orders starting with numbers other than 001 or 1001 unless you are re-ordering additional tickets from a previous order within 60 days.
Stub Size	Raffle Tickets	Different stub sizes are available by special order. there is a $20.00 set up fee per order.
Composition	Window Tags	Custom window tag orders have a composition charge of $25.00 and include up to 6 lines. Additional lines at $2.00 each. This charge is incurred on the first order only.
