If right hand stub is a coupon or requires special logos, you may need to supply us with copy. Although we maintain 1,000's of logos and trademarks in our library we cannot have all local or regional styles. Art should be good black and white copy. Photocopies or colored copy may not be reproducible. You may also e-mail us with attachment of the following files: pcx, wmf, tiff, corel 4 or 8, MS word (doc), or .bmp.
If you have a coupon (such as $1.00 off a pizza) you must FAX/e-mail a copy of the coupon with phone number of the responsible person providing the coupon value. We will contact the providing person (business) to verify use of coupon.

Custom Forms: Before we process your credit card information, we will set your form, show you a proof and get your final approval. Once you have given us your final approval we will then process your card information and proceed with the production and shipment of your order.
